Tattoo Booking Form
Book an appointment for November of 2019!
I work at Powell Tattoos in Portland, OR and we are CASH ONLY!
PLEASE READ BEFORE SUBMITTING
Upon mutually agreeing on the appointment date provided, a $50 deposit is required to secure your appointment - which you can send through PayPal (Friends & Family option) or in-person at the shop.
After submission, I will respond back to you as soon as I am able. Submitting an application is not a guarantee that you will be tattooed. However, if I do not hear back from you accordingly after personally confirming through e-mail, the date desired may become unavailable. Deposits are required to hold a date once mutual confirmation has been established. All dates and flash designs are first come first serve.
Deposits are non-refundable. I will put it toward your newly rescheduled appointment f you are able to call/e-mail BEFORE the appointment (24hrs) and re-schedule. Your deposit counts toward the final cost of your tattoo as well! If you cannot make your appointment for ANY reason, please don’t be nervous... be courteous and give me a heads up!
If you do a no-call/no-show, your design will be up for grabs to the public and you will not get your deposit back.
If I have to reschedule you more than two times, I will ask for another deposit.
Designs: I am currently primarily interested in illustrative blackwork (please check out my instagram if you have not for a better idea of my style: @heavyblossom). I am now open to pops of color like pink and red!
I am able to do in-person consultations before booking if you wish/if I have time. I do not show sketches beforehand and will have a final design drawn up the day of your appointment. I am pretty flexible when it comes to adjusting elements in a design. However, I do work best with a loosely based idea that allows for more creative flexibility and artistic freedom! Trust! If I have to re-draw the initial design more than 2 times, I will charge an additional $25.
